Take a look at the Trip Advisor Lombok forum and comments about the guide Eko. You could probably work with Eko to put together a similar itineray but be more flexible around weather and energy levels:) He has a facebook page and email address so you can do some organising before you get there - that way you won't waste a day on arrival sorting out an itinerary. If Eko is not available I also have a relative who organises tours, but he is maybe not as organised as Eko in terms of email addresses etc so hard to pre-arrange tours - however once you are there he is also a great guide.
Also, if you go down to Kuta Lombok, the owners of Mimpi Manis homestay have a car and also can arrange tours for you - Gemma is English, so no communication troubles there.
Also, in regard to your transfer from Bali to Lombok. There are other options that via Sanur and fast boat. Not that Sanur fast boats are not a good way, but to fill you in:
Ex Sanur (and probably Benoa port) to Lombok is the longest sea journey of all the fast boat options. It takes 2.5 - 3 hours by sea, and the sea can be very rough. The attraction is that there is less travel from Denpasar to get to the boat.
If you prefer to go by fast boat, then a better option in my mind is ex Padang Bai, it involves a 1.5 hr drive to Padang Bai, and a 1.5 hour boat trip. Gili Cat is the best operator. The total travel time is the same, but half of it is by land and more comfortable.
However, be aware that they advise pregnant women and people with bad backs not to take fast boats to Lombok - so that says it all really. It can be
extremely bumpy. If you do go by boat make sure you sit as far to the back as possible, and toward the middle, that way you get the least movement.
The alternative is to fly. It is actually cheaper than fast boat. 3 airlines operate several flights a day to Lombok, departing Denpasar domestic airport and arriving Mataram airport 25 minutes later. It sure is my preferred method. Most people who go by boat are going directly to the Gilis, so they find fast boat from Bali more convenient as they then don't have to get from the airport over to the Gili islands.
If you would like some accommodation recommendations let me know what sort of places you like to stay (budget in Lombok means no hot water and fan cooled, or do you need a/c and hot water with swimming pool, do you want 4 star service, do you want small boutique or large chain, etc, and locations).
Planning the trip is half the fun
